The Utah Name Change Process

1. Social Security Card

Step one of your name change process is filling out the SS-5 form. Once completed, decide if you want to file by mail or in person. 4. Utah Driver’s License

The fee is $18 if you are just changing your name. If you are also renewing your license, the fee is $25.00.

After processing your information, your new Utah driver’s license will be issued the same day you apply!

5. Utah Voter Registration

Changing your name on your Utah voter registration is the last state-level update you will need to file. Simply complete the mail-in voter registration form and send it to your county clerk’s office.

6. ALL of Your Creditors and Accounts

Now that you’ve filed your federal and state name change forms, it’s time to notify your creditors, memberships and accounts. Start with your employer and banks. Usually they will require you to complete a form in-person, and show your certified marriage certificate. Next, notify your credit cards, professional license boards, mortgage providers, loan providers, and utilities of your new married name.
