Once you file legal name change forms, you do not have the opportunity to change them without a legal name change order from the court system or a divorce decree. Yikes! Tip 3: File the SS-5 form first.

Many states, banks, and service providers require newlyweds to file for their name change with a certified marriage certificate AND an updated Social Security Card in their new married name. Did you know that the Social Security Administration takes approximately 2 weeks to issue new Social Security Cards? Your new card will arrive in the mail and will show your new married name. Your Social Security Number will not change. So, no need to memorize a new one and update it everywhere!

Tip 4: It’s free!

There is no fee associated with Social Security name change. You simply complete the SS-5 form and file in-person or by mail with your certified marriage certificate and photo ID (such as your driver’s license or U.S. Passport.) Then, your certificate and ID will be mailed back to you with your new Social Security card in your new married name!

Social Security Name Change

The first place to change your name is with the Social Security Administration. You will need to complete the SS-5 form and file it by mail or in-person with your certified marriage certificate and photo ID. Your social security number will not change, but a new card will be issued in your new married name. This card will arrive by mail approximately two weeks after you file the SS-5 form.

IRS Name Change

Next up is notifying the IRS of your new married name. Complete the IRS 8822 form and mail it to the appropriate IRS office address listed on the second page of the form (which is based on your state of residence.) Please note that the IRS offices do not have street addresses. You simply mail to the abbreviated address listed on the form. Filing the IRS 8822 form is free, and ensures that when tax time arrives the IRS knows who you are, what your married name is, and where to send your tax returns.

U.S. Passport Name Change

The last federal office to update your name with is the State Department. Select the appropriate passport form to complete based on your current passport status. You will need to submit your current U.S. passport when you file, so be cognizant of any up-coming international trips. The State Department is taking approximately 5 weeks to issue new passports. If you need yours faster, you can pay an additional fee to have your passport processing expedited.

Massachusetts Driver’s License Name Change

In Massachusetts, you must file for your new driver’s license with your new Social Security card showing your new married name. This is why filing the SS-5 form is at the beginning of our list! Along with your new Social Security card, you will also need to bring your completed driver’s license application form, certified marriage certificate, current state driver’s license or photo ID, proof of residence (ex: utility bill, paycheck) if your address has changed, and $25 for the application fee. Your new Massachusetts driver’s license will be issued the same day you file for it.

Creditor Name Change

Once you’ve filed all of your federal and state-level name change forms, you will need to notify all of your banks, credit cards, insurances, medical providers, employers, insurance providers, mortgage providers, membership companies, magazines, etc. Basically any company that sends you anything by mail, or bills you in your maiden name will need to be notified of your new married name. The notifications can be done by phone, mail, or fax (yes fax machines still exist) depending on the entity you are contacting.

Applying for your marriage license in Arizona is really easy. Licenses are issued the same day you file. And they are valid for 12 months. Arizona Certified Marriage Certificate

Approximately 2 weeks after your wedding, your certified marriage certificate will be issued and mailed from the County Clerk’s office. This certificate is the legal document “aka proof” of your marriage. You will need to use this certificate to file your name change paperwork. We highly recommend requesting an additional 1-2 certified marriage certificates. This gives you the option to file multiple forms at once, and streamline your name change process.

Social Security Name Change

The first federal name change form to tackle is the SS-5. Once you’ve completed the form questions, you will need to file in-person or by mail with your certified marriage certificate, and state driver’s license or U.S. Passport. Your new social security card with your new name will be issued within two weeks. Your name will change on the card, but your social security number will stay the same.

Next, it’s time to handle the IRS 8822. Complete this free form to ensure that the IRS is notified of your new married name. Thus preventing any tax return confusion! Expert tip: The Decedent is an individual who has passed away and left you funds. It’s an IRS question we answer all the time at MissNowMrs.

U.S. Passport

To change your name on your U.S. Passport, you will need to complete the DS-11, the DS-82, or the DS-5504. You can select the appropriate form based on your current passport status. Then, locate the supporting documents necessary for filing your passport forms. These include a certified marriage certificate, 2 passport photos, and your current passport or photo ID. The State Department typically takes 5-6 weeks to process and mail your new passport in your new married name. If you submitted your previous passport in your maiden name, it will be returned to you with two small holes punched in it.

Arizona Driver’s License Name Change

When updating your name on your Arizona driver’s license you should complete the Arizona Driver’s License Application form. Wait 48 hours after filing the SS-5 form with Social Security before filing for your new driver’s license. Then, bring your completed form, current license, certified marriage certificate, proof of residence (ex: utility bill or paycheck,) and $12 for the filing fee. Your new Arizona driver’s license will be issued the same day you apply for it. Woohoo!
